Hong Kong issues a notice after a fatal accident during cargo operations

The Marine Department of the Government of Hong Kong has issued a notice after a fatal accident during cargo operations.

Incident Summary

A fatal marine industrial accident occurred during the unloading of brick cargo from a multi-purpose river trade cargo vessel to a locally licensed dumb lighter at the New Yau Ma Tei Public Cargo Working Area in Hong Kong.

While unhooking cargo at the berth, a stevedore attempted to untangle canvas slings, resulting in a partial cargo collapse, ultimately leading to his fatal injury.

Contributing Factors

The investigation revealed that the injured stevedore lacked safety awareness. He did not wear protective equipment such as a safety helmet and safety shoes and failed to maintain a safe distance from the operational derrick.

Additionally, the lifting appliances of the lighter were not tested and examined as required by the Merchant Shipping (Local Vessels) (Works) Regulation (Cap. 548I).

Lessons Learned

To prevent similar incidents, the vessel and lighter owners, as well as the cargo owner, must improve safety training for cargo handlers and increase their safety awareness.

The lighter owner must comply with the Works Regulation by conducting periodic tests and examinations of lifting appliances.
